FIELD TRIP PERMISSION

During the school year, Oakwood Elementary School students will be participating in various field trips. Under the supervision of our teachers, they will visit various sites of educational value.

Please sign below indicating that you give permission for your child to participate in these field trips. This form will be on file and will suffice for the entire year.

We will send notes home prior to each field trip specifying the date, time, and any lunch, etc. that might be required. Please be alert to these messages, which notify you that your child will be transported from the school premises.

If you do NOT give permission for your student's photo to appear in school publications, including the website, promotional materials, or yearbooks, please visit the office to sign an opt out form.

REDUCED LUNCH APPLICATION

If you prefer to fill out the online Reduced Lunch Application, please use the link. Paper copies are available at each school if preferred. Meal prices per day are as follows:

Breakfast:

$3.00 full pay (students)
$0.30 reduced pay
$4.00 (adults)

Lunch:

$3.70 full pay (elementary students)
$0.40 reduced pay
$5.30 (adults)
